File a Petition to
Arbitrate a Legal Malpractice Claim

The two steps to file a petition to arbitrate a legal malpractice claim through the Attorney/Client Arbitration Board are:

  1. Read carefully the Information Sheet on the Arbitration Service.
    1. Information Sheet
    2. Notice to Clients
    3. Rules of Procedure
  2. Submit the original plus two copies of the following three forms along with a filing fee. Incomplete petitions will be returned. You should also keep a copy for your own records.
    1. Request for Arbitration
    2. Agreement to Arbitrate
    3. Statement of claim

Proceed to step one, reading the Information Sheet.
